Thank you for considering Southern Worcester County Educational Collaborative (SWCEC) as an option for your student who has extended assessment or interim placement needs. Southern Worcester County Educational Collaborative can provide an alternative assessment program for students who present with emotional and behavioral challenges that directly impact their academic performances, school attendance, and peer relations. 

The extended evaluation is designed for students ages 5 through 22 who may be receiving special education services, or 504 services.  The extended evaluation program will provide the following in a highly structured, therapeutic milieu: 

  1. Classroom instruction based on the learning standards of the Massachusetts Curriculum Frameworks provided by MA licensed teachers in a small group setting;

  2. Weekly individual and/or group counseling services;

  3. Crisis intervention;

  4. Positive behavior interventions and support (PBIS);

  5. Related services per student’s IEP;

  6. Age appropriate vocational and transitional support;

  7. Daily communication system between school and parent/guardian.

Please use the “Submit Extended Evaluation Questionnaire” button below to complete the SWCEC Extended Evaluation Questionnaire. Upon completion of the form you will be asked to submit the following student information:

  • Current IEP or 504 Plan

  • Most recent evaluations

  • Progress notes

  • Transcripts (if relevant)

  • Behavior/Discipline Reports and/or Behavior Plan

  • ELL information (if relevant)

  • NOTE: Student Health Records will be required prior to a start date for accepted students.

 The Extended Evaluation Referral Process:

  1. Complete the SWCEC Extended Evaluation Questionnaire and Options Checklist

  2. Submit relevant student documentation that may include: signed release of information, current IEP or 504 plan, most recent evaluations, progress notes, transcripts if relevant, behavioral/discipline reports and/or behavior plan, ELL information if relevant. Student health records will be required prior to a start date for accepted students.

  3. Upon receiving the completed referral packet the SWCEC team will review the documentation and contact you with follow up questions as needed

  4. SWCEC will schedule an intake meeting/tour with the student/parent/guardian/district

  5. A start date, a midpoint progress review meeting, and an end of evaluation period IEP Team meeting will be scheduled.
